Disk I/O tracing for Linux 2.6 kernels

This is a kernel patch and a couple of user-space programs that allows for tracing and logging log-level disk I/O requests in Linux 2.6. It should work on most block devices, but I tested only using SCSI.

Currently, we only support 2.6.9-mm1.
